When you arrive at HOPE for your first appointment
Please have with you any of the following appropriate items:
- X-ray films: If you have had an x-ray, CT, MRI or mammogram from your primary care physician relating to your health issue, you need to bring your x-ray films to this appointment. X-ray films are different from x-ray reports—your doctor at Hope will need to have the actual films, which look like large photo negatives, in order to provide a complete consultation.
- You will need to show your insurance card or cards and authorization for this appointment, if required by your insurance. If you have any questions about insurance or finances, please call and ask to speak to someone in our insurance department.
Each of our patients has special needs and it can be very hard to predict how much time each person will spend with the Doctor or Nurse Practitioner. We make every effort to run the office schedule on time, but must also give each patient the time needed with the doctor. Please feel free to call us prior to your appointment if you have any questions.
Special Needs Patients
If you have special needs due to any type of disability, please let us know when you schedule your exam. We will allow extra time and consideration for your needs.
